Maranatha has a school nurse on campus to manage student health needs. All required health records should be emailed to Nurse Bobbie Ehlen or faxed to 763-315-7294.
Physicals & Immunizations
Returning students entering Grades K, 7, 10, and 12 must provide an updated Health or Sports Physical and Immunization form each year. Non-medical exemptions require a notarized parent signature. The MN Department of Health immunization chart is linked on the Health page.
Sports Physicals
The MSHSL requires a new sports physical every three years. The physician form is available on the Health page.
Medications at School
An Authorization for Administration of Medication form — signed by both a parent and healthcare provider — is required every school year for all medications, prescription and over-the-counter. Upper school students (grades 9–12) may carry inhalers with a self-administration form on file. All other medications must be stored in the health office.
